The midwife worked at General Hospital. Yvonne Tasker, a nurse since 1991, has been suspended following the death of an unborn baby hours before it was assistantly born

A midwife suspended after the tragic death of a baby under he care had "no evidence of remorse", a panel found.


Yvonne Tasker was a midwife at the General Hospital when the patient was put in her care in 2017. Sadly, the patients' child died before she gave birth a few hours later.




Tasker, a nurse since 1991, attended a Nursing and Midwifery Council (NMC) hearing this month where she faced 21 charges relating to the patient’s care, most of which related to poor time keeping and a lack of communication. Although they conceded Tasker "recognised some of her failings" it did not cover all aspects of the charges and there was "no evidence of remorse."

After the hearing, the panel said: “Midwives occupy a position of privilege and trust in society and are expected at all times to be professional and to maintain professional boundaries. Patients and their families must be able to trust midwives with their lives and the lives of their loved ones.

"To justify that trust, midwives must be honest and open and act with integrity. They must make sure their conduct at all times justifies both their patients’ and the public’s trust in the profession.

“The panel finds that Patient A and her baby were put at risk as a result of Ms Tasker’s misconduct. The panel further found Ms Tasker’s misconduct had breached fundamental tenets of the midwifery profession and therefore brought its reputation into disrepute.”

Patient A was admitted at the hospital in October 2017 following an uneventful pregnancy for a labour assessment at 40 weeks. She was found to be in established labour but risk factors meant her labour should have been consultant-led, rather than midwife-led and a care plan should have been documented by the Obstetric Team (OT).

The panel also heard Tasker incorrectly categorised the patient as low risk and artificially broke the patient's water. She claimed she was given verbal consent which the mother-to-be refuted.

No foetal heart rate could be detected but Tasker took 15 minutes to alert the team when it should have been treated as an emergency. The consultant obstetrician confirmed the patient’s baby died in utero just after 11pm that night.

The panel decided not to strike Tasker off as it was felt it was not proportionate due to the fact the misconduct happened during one shift with no evidence it had happened before or since. Instead, it issued a one-year suspension order which she has 28 days to appeal the order before it takes effect.

A spokesperson for Northern Lincolnshire and Goole NHS Foundation Trust said: “We’ve fully supported and participated in the NMC investigation. As a result of an internal investigation, we’ve introduced new systems, assurance processes and additional training for staff.

We would again like to offer sincere condolences to the family for the sad loss of their baby.”
